What's the difference with the telescopic scraper?

This one has a brush in addition to the scraper: it sweeps the snow before scraping the ice. The telescopic model in the same range is a scraper only, lighter and longer, but it doesn't sweep.

Does the brush scratch the paint?

Not if it's used on fresh snow, without pressing down. Scratches come from dirty snow pushed with force, or from the scraper dragged across the bodywork.
